In case of loss of membership card, member should immediately inform the Club’s management on telephone and lodge a Police FIR to avoid its misuse.

For duplicate membership card FIR copy is required to be uploaded in case card is lost.

In case card is damaged, picture of damaged cards will be uploaded.

Only JPEG files to be uploaded (Where required).

Size per attachment must not be larger than 1 MB.

Cards will be issued after 8-10 working days after submission of this form.

Member / household members or authorized persons, may collect membership cards during working hours from club’s office.

Applicable fee will be charged in monthly bill.

All the Bye Laws of respective Club will be applicable.
